Instructions and Help about fillable genogram template for word

In this presentation I will discuss and demonstrate how to draw Gina Graham's a map of the family commonly used in interprofessional practice as a documentation tool for family structure I will start by defining some commonly used terms and show you the most common standard symbols and lines as well as what data can go on a genogram so what is a genogram it is a diagram of the generations we prefer to get at least three generations as that gives a more complete picture start with the children as the first generation second generation is their parents and cynical and third is their grandparents so here it is again first generation second generation and third generation some terms you will need to know include family of origin this is the family from which a person originated, so it would be for me my brothers my parents my grandparents the family of procreation is a couple with their children, so it would be me my partner and our children it is important for loan parents when you are drawing their genogram to include the children's other parent think of the genogram as a diagram of the children's ancestry the index family or person the family or person that the nurse is working with is shown in a couple of different ways if it is an indexed person you do a double symbol for that person or for the index family you circle the entire family so here is how you would show the index family the nurse is working with the parents and there are three children in this family and the client the nurses most concerned with is the middle child, so the ten-year-old son is the index person symbols will indicate the gender of a person so male female lesbian are gay sexual orientation the placement of dates can be important so date age and death date have particular placements a double square or a double circle would indicate a person who is identified as the patient or the index person in that family lines can also differentiate marriage living together separation divorce and even remarriage after divorce for children you have lines that will indicate foster children versus adoption versus biological still birth miscarriage and abortion, and you can identify the difference between fraternal and identical twins by adding a line here the triangle is most commonly used here to indicate pregnancy you will find some sources in nursing textbooks that refer to this symbol as abortion which is really the very opposite meaning, so I would encourage you to have a key and indicate what exactly you mean when you use this symbol we can also shade symbols to indicate types of disorders and health problems that people have I would strongly encourage also writing down what specific problem there is so that beside this person for instance with a serious mental or physical problem I would expect to see written malignant hypertension or paranoid schizophrenia or whatever the specific health problem was we can also use lines to show the quality of relationships between people these...
